Physical Night: May 12th | 4:00 PM โ 7:00 PM @ APHS.
Athletic Clearance Opens: May 18th (via Home Campus).
Validity: All forms and certificates must be dated 5/12/26 or later to be valid for the new school year.

New Forms Required: You must use the NEW FHSAA physical packet (revised 2/26). Physicals MUST be completed on this specific form.
Where to get one: * Dr. Lackeyโs Office (Sebring): Free physicals all year long (Appointment required).
APCC: Free physicals (Appointment required).
ECG Requirement (New State Law): All incoming freshmen and students who have NEVER played a high school sport must complete an ECG on the new EL1 form.
Note: Current athletes who have already played an FHSAA sport in high school do not need a new ECG.

Do NOT turn in physical packets to the school. Parents must keep the hard copies and upload everything digitally.

Uploads (Part 1): Upload Page 4 only of your Physical (EL2) and your 4 NFHS Video Certificates (Concussion, Heat Illness, Sudden Cardiac, and Sportsmanship).
Uploads (Part 2): Upload Emergency Treatment form, drug testing, parent contract, ECG/EL1, birth certificate, proof of residence, government ID, and insurance card.
